PEER 100 Assistance Request

PEER 100 Assistance - Assistance with mental health prevention and intervention services for first responders struggling with the impact of their public safety profession. Up to $5,000/person, lifetime allotment. 

*This request must be submitted on behalf of the individual requesting support by an EAU/Peer Support representative, first-level supervisor, H/R supervisor, chaplain, mental health and/or medical provider (not the provider the funds will be paid to). The request must come from the submitters email/submittable account. 

We do not reimburse individuals, we pay the providers directly. 

Examples of support services; hyperbaric oxygen therapy, Amen Clinic, stellate ganglion block, ketamine treatment, intensive outpatient services, and inpatient treatment. Individuals can self-submit for Alpha-Stim, BrainTap, Cereset, and WatchPAT Home Sleep Study (see additional form).
